Essential Functions of Job:

● Using clinical knowledge, support health coaching team in the review of risk assessments and claims data; offer clinical advice in the development of individual coaching plans, including inviting members to participate in 1:1 chronic care management services ● Using clinical knowledge and knowledge of Whirlpool’s benefit plan design help direct employees to local primary care physicians and specialists, and centers of excellence that are appropriate for the conditions requiring care ● Assist, as needed, with large biometric screening events and retesting ● Provide clinical guidance and assistance in the benefit appeal process ● Assist as needed, with planning and promoting flu shot clinics ● Using clinical knowledge, develop and maintain chronic condition management programs in support of health coaching plans (i.e. Sweet Success Diabetes program) ● Post/distribute chronic condition related communication materials ● Provide clinical knowledge and support for local Wellness Champion networks ● Using clinical knowledge, support health coaching team in improving the culture of health (i.e. café/vending options, establish walking paths, etc) ● Support specific chronic condition care related initiatives that align with improving HealthWorks Scorecard as directed by supervisor ● Answer benefit and clinical questions specific to chronic conditions or conditions requiring complex/high cost care (i.e. imaging, surgery, specialty drugs, etc.) ● In collaboration with Health Coach, meet with site contact or HR Director on a monthly basis to share program plans, metrics, monthly reporting and other wellness related initiatives • Other duties as assigned by supervisor • Ability to report to the office

Qualifications:

• College Degree required: Associates or Bachelors of Science in Nursing • Current Arkansas nursing license required • Effective organizational and prioritization skills • Excellent oral and written communication skills • Basic knowledge of Microsoft Office software applications (Word, Excel, Outlook, Access) • Ability to work in a fast paced multi-tasking environment • Strong interpersonal skills with ability to function as a positive team member • Strive to meet program deadlines • Knowledge of medical claims administration and employee benefit plans preferred • Clinical experience and/ or a Managed Care background preferred • 1-2 years of health promotion/wellness or other health-related program planning and delivery experience preferred • Minimum of Five years clinical nursing experience preferred
