If you are joining a clinic that is already participating with HealthScope Benefits, you can print a provider application to complete and mail to us. After credentialing occurs, you will be rolled under the participating clinic agreement that currently exists.
If you are not joining a participating clinic, please contact our Provider Relations department to request a contract packet. Do not complete the online application without a valid agreement. Thank you.
